Noise - whining or grinding? Worn gears or dry gears probably. Try some appropriate grease, if still noisy they're worn. The diagram should be like this Support :: Hornby Hobbies
By the look of things decidedly user unfriendly. Looks like you pull the buffers off, remove the body shell, remove the cover to the motor, remove the motor, squirt grease on the gear train, replace motor, bit of grease on the worm, reverse steps. Or remove the bottom plate, squirt grease on axle gear. Going from the underneath means pretty much removing everything, although if you work upside down and don't disturb the wheels and rods it should be OK. Watch the electrical pick-ups when reassembling.
Spot of oil in the end of a wire to the axle bearings.

I wasn't thinking that old. Toothpaste or gear polishing paste sometimes helps, which would mean a complete strip down. Just live with it, or start building your own chassis.
